Answer: A W-9 form is used to gather your tax information such as your TIN or Taxpayer Identification Number, and that can be your social security number for some, but they also have an area for the "Employer Identification Number" for those who have one. This is for a person who will be paying you some money usually but not always in excess of $600.00 in a year.
The people who need to fill out the W-9 have to submit it to the payer who will send them a 1099 form with the amount of money that was paid out to them during the year in the required amount of time early in the following year.
The person who got paid is a sole proprietor, etc., and they got paid during one year, and at the beginning of the next year they will receive a 1099 form for use in filing taxes. There are other uses, as well.
When you sign the form, it states "Under penalties of perjury... etc." so be very accurate.
The W9 Form states that:
"A person who is required to file an information return with the IRS must obtain your correct taxpayer identification number (TIN) to report, for example, income paid to you, real estate transactions, mortgage interest you paid, acquisition or abandonment of secured property, cancellation of debt, or contributions you made to an IRA." and more.
Of the 4 pages, only one is filled out, and pages 2, 3, 4 are instructions that are understandable.
